JOB PURPOSE:
The Clinical Pharmacist is responsible for providing a variety of direct patient care and service delivery functions remotely and/or from our call centre. Working together with the leadership team, other pharmacists and support staff, the Clinical Pharmacist plays an integral role in delivering our medication management programs to patients, contributing their clinical expertise to program development and delivery, monitoring the quality and effectiveness of our programs, and participating in the development and ongoing review of our clinical algorithms. The Clinical Pharmacist performs his/her duties according to the Standards of Practice and Code of Ethics for Pharmacists, regulations governing the practice of Pharmacy and patient privacy, and always puts patient safety and wellbeing first.
The Clinical Pharmacist will resolve issues, make decisions, and escalate issues appropriately as it pertains to clinical patient care and program delivery.
ESSENTIAL JOB FUNCTIONS:
The job functions of the Clinical Pharmacist may include all or some of the functions listed below. The specific functions of an individual Pharmacist may change over time as the company grows, new programs are introduced or new roles are required to meet the changing needs of the organization.
- Provides direct patient care as a consulting Pharmacist in our telehealth care programs:
o Delivers a variety of programs such as MedMonitor, MedCheckUp or others
o Consults with patients, prescribers and other healthcare providers about medication therapy
o Optimizes medication therapy to meet the needs of patients
- Contributes to the ongoing validation, review, revision and development of clinical algorithms used in HumanisRx’s clinical programs
- Provides input on the development and quality monitoring of care programs
- Provides input on the work flow processes for program delivery
- Collaborates with other members of the program team, such as Pharmacy Technicians and support staff, to ensure proper prioritization and completion of work to support the successful achievement of clinical and organizational goals and objectives
- Other duties as assigned from time to time of a clinical or administrative nature
QUALIFICATION REQUIREMENTS:
Must be a Registered Pharmacist in Saskatchewan, Manitoba, Nova Scotia, or New Brunswick, and a member in good standing with their respective College/Regulatory body.
Bilingualism (English/French) would be required for candidates applying from New Brunswick.

EDUCATION:
A minimum Bachelor of Science in Pharmacy degree; preference given to Doctorate of Pharmacy (PharmD) degree or similar advanced credentials.
EXPERIENCE:
A minimum of 3 (three) years’ experience working as a Registered Pharmacist in a clinical practice setting, to include:
- Demonstrated experience providing direct patient care including medication therapy reviews, patient consultations and interventions with other healthcare providers to improve patient health
- Demonstrated experience of collaboration within a team setting
- Demonstrated clinical skill and expertise
- Demonstrated commitment to ongoing continuing education in clinical fields from accredited and approved educational programs or academic institutions
- An equivalent combination of education and experience may be considered
OTHER SKILLS and ABILITIES:
- Demonstrated comfort with technology, both hardware and software
- Effective people skills and the ability to build trust and relationships
- Advanced skills in balancing multiple priorities in a dynamic and fluid environment
- Ability to communicate effectively and professionally in English and French through written, verbal, and interpersonal skills applied when interacting with patients, caregivers, healthcare providers and co-workers; successfully conveying and exchanging information in a positive manner
EXPECTED PERCENT EFFORT:
- Core Duties 90%
- Quality Assurance / Quality Improvement 10%
PHYSICAL DEMANDS:
This position is cognitive in nature and will present physical demands requisite to a position requiring: hearing, seeing, sitting, standing, talking, and walking. The telehealth care model requires lengths of time talking on the telephone and documenting in a computer. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. WORK ENVIRONMENT:
This position works in a hybrid office/home environment with moderate noise, including some work situations that could present repetitive muscular, vocal and vision strain.
